Halcinonide

The recurrent attacks of chronic inflammatory skin diseases such as allergic dermatitis and eczema are long-standing clinical treatment pain points in dermatology, and topical glucocorticoids are the first-line medications for these diseases. As a typical variety of high-potency fluorinated glucocorticoids, halcinonide binds to glucocorticoid receptors, inhibits the release of inflammatory mediators, reduces inflammatory cell infiltration, and exerts multiple effects including anti-inflammation, anti-proliferation, anti-allergy and immunosuppression. It is clinically mostly used for the treatment of non-infectious inflammatory skin diseases such as contact dermatitis, neurodermatitis, seborrheic dermatitis, eczema, psoriasis, lichen planus, etc., and is particularly suitable for patients with intractable and hypertrophic skin lesions who show poor response to other medium-potency hormones. Local topical application of halcinonide has a fast onset of action and long-lasting effect, with low transdermal absorption, so the risk of systemic adverse reactions is controllable.

The global market size of topical glucocorticoids is growing steadily, and the overall size had exceeded USD 8 billion in 2023, with high-potency hormones accounting for approximately 23%. The annual sales of halcinonide preparations at the terminal of public medical institutions in China have remained above RMB 300 million all year round, with an annual growth rate maintained at around 5%. The market is dominated by generics, with more than 30 domestic manufacturers. Commonly used dosage forms such as ointments and solutions have been included in the National Medical Insurance Category A Catalogue, and the demand in the primary market is stable. The localization rate of halcinonide API is relatively high, and a number of domestic enterprises have obtained production approvals with sufficient production capacity, which can fully meet the production needs of downstream preparation enterprises.

The original research enterprise of halcinonide is Bayer, with the original brand name "Halog", and its core compound patent expired in 1980. The main dosage forms approved for the original product include ointment, cream and topical solution, among which the 0.1% strength ointment, cream and solution have all been included in the FDA Reference Listed Drug Catalogue, and the original preparation has not been imported and marketed in China. In terms of domestic API registration, as of now, halcinonide APIs from more than 10 enterprises have been registered with CDE, all with the status of "A", and can be legally marketed and used in China; on the preparation side, multiple dosage forms including halcinonide ointment, halcinonide solution and halcinonide cream have been approved, among which relevant varieties from several enterprises have passed the consistency evaluation.
