Cladribine

In the clinical treatment of hematological malignancies, purine nucleoside analogs have always been an important medication option. As a representative of the second-generation purine analogs, cladribine inhibits the activity of adenosine deaminase, interferes with cellular DNA synthesis and repair, and selectively induces apoptosis of lymphocytes in both proliferative and quiescent phases. Its clinical indications cover hairy cell leukemia, relapsed/refractory mantle cell lymphoma, Waldenström's macroglobulinemia, etc. It is also used for immunomodulatory treatment of some immune diseases, providing a low-toxicity and high-efficacy treatment path for patients with B-cell-related hematological diseases, especially suitable for elderly patients who cannot tolerate high-intensity chemotherapy.

The current global market size of cladribine has exceeded USD 320 million, and the compound annual growth rate from 2023 to 2029 is expected to remain at 5.8%. The growth momentum mainly comes from the increased diagnosis and treatment rate of indolent lymphoma and the expansion of indications in the field of autoimmune diseases. In terms of the domestic market, with the implementation of precise diagnosis and treatment specifications for hematological tumors, the annual average growth rate of in-hospital sales of cladribine preparations in the past three years has reached 22%. At present, only 3 local enterprises have obtained generic drug approvals, the competitive landscape is relatively loose, and it has not been included in the scope of national centralized volume-based procurement, so there is still a large gap to fill in the supply of active pharmaceutical ingredients.

The original developer of cladribine is Janssen Pharmaceuticals, a subsidiary of Johnson & Johnson, with the original brand name Leustatin®. The core compound patent in the US market expired in 2006, and the compound patent in China also expired in 2006. The dosage form approved for the original product is injection, with a specification of 10mg/10ml, which has been included in the FDA Reference Listed Drug Catalog and China's *Catalog of Reference Preparations for Chemical Drugs (Third Batch)*. As of now, 6 domestic enterprises have obtained Class A status for their cladribine active pharmaceutical ingredients through CDE registration, and another 11 enterprises have obtained marketing approval for their generic cladribine for injection.
